As flu season quickly approaches, NOVA’s Human Resources has coordinated flu shot clinics for NOVA employees. Clinics will be hosted by Walgreens Pharmacy. Onsite clinics have been scheduled at the following locations this year:

The clinics will be open to all COVA CARE, COVA HealthAware and COVA HDHP participants and will operate on a walk-in basis. In advance of your attendance, please complete Sections A, B (1-8) and C of the Walgreens Vaccine Administration Record (VAR) Informed Consent form in the Flyers section of this edition of the Intercom.
You should ensure that you indicate the name of your health plan and plan ID number (located on your state health plan ID card) by writing it across the top of the form. Bring your completed, signed and dated form, along with your health plan ID card and a valid picture ID with you to the clinic for verification purposes.
Note: Kaiser Permanente HMO participants may receive free flu shots at their doctor’s office and participating Kaiser medical centers.
No other immunizations will be administered at the clinic.
