Information on Required Forms to Enter College Buildings

As we begin the fall semester, the Return to NOVA Advisory Group (RNAG) wants to update you on the two forms that allow faculty and staff to access college buildings. Form 105-211 (Access to NOVA Facilities During Remote Work) and 105-212 (Supervisor Notification to Employee of Return to On-Site Work). Approval through one of these two forms is required before a faculty or staff member comes on site to a NOVA campus or leased facility. Below is a brief overview of how and when to use each form.

Form 105-211: Access to NOVA Facilities During Remote Work. This form is for those that need to come on site for a “grab and go” visit, such as picking up items in your office or visiting IT to fix a college laptop. The form can be accessed online at https://eforms.nvcc.edu/FormsLibrary/105-211/. The submitter will receive a notification once the request is approved. Please note that grab and go visits must be made during standard building operating hours and a new form is required for each visit.

Form 105-212: Supervisor Notification to Employee of Return to On-Site Work. Supervisors must submit a 105-212 for any employee returning to work on site. The form can be found in the eForms library at https://eforms.nvcc.edu/FormsLibrary/105-212/. The 105-212 includes links to COVID19 resources, information and trainings that must be reviewed by the employee prior to returning to work on-site.

Employees must have approval through a 105-211 or 105-212 prior to entering a NOVA operated building. Finally, please remember to take your health screenings in GoCanvas and stay home if you are sick!
