Legal Resources Open Enrollment, Oct. 1 through Oct. 31

The open enrollment period for participation in the Legal Resources Plan begins October 1 and ends October 31, 2020. The plan provides limited legal services to benefits-eligible employees, their spouse and qualifying dependent children at the group rate of $18.00 per month.  Plan features include no annual usage limitations, co-pays or deductibles for covered services. Enrollment forms may be submitted by fax to (703) 323-3155, or by secure e-mail to Michael Gadway on the benefits team at mgadway@nvcc.edu.

Important Notes:

• A payroll deduction of $9.00 per pay period will begin with the Dec. 16, 2020 pay date for coverage effective Jan. 1, 2021.
• Current participants need not re-enroll for continued participation.
• Both the FBMC form and the Legal Plan Enrollment Form must be received in order for elections to be complete.
• To cancel participation, simply fill in name, state ID number, sign and date both forms and write “CANCEL PARTICIPATION” across the bottom of each form.
