Legal Resources: Open Enrollment Begins Oct. 1
The open enrollment period for participation in the Legal Resources Plan begins Oct. 1 and ends Oct. 31. The plan provides limited legal services to benefits-eligible employees, their spouses and qualifying dependent children at the group rate of $18.00 per month ($9.00 per pay period). Plan features include no annual usage limitations, co-pays or deductibles for covered services. Click on the links below for a detailed plan overview as well as the required Legal Plan Enrollment Form and the FBMC Payroll Authorization Form. Completed forms may be submitted to Michael Gadway by fax at 703-323-3155 or by secure e-mail to mgadway@nvcc.edu.
Important Reminders:
- A payroll deduction of $9.00 per pay period will begin with the Dec. 16, 2021 pay date for a Jan. 1, 2022 coverage effective date.
- Both the FBMC Payroll Authorization Form and the Legal Plan Enrollment Form (found within the link below) must be received in order for elections to be complete.
- You can only cancel participation during open enrollment. Simply print your name, sign and date both forms and write “CANCEL PARTICIPATION” across the bottom of each form.
- Current participants do not need to re-enroll for continued participation.
- Participation in the program is open to all salaried employees.
