Introducing a New Electronic Form for Condolence Requests

NOVA is introducing a new electronic form to notify Human Resources and the Office of the President in the event of the passing of a current or former NOVA employee, an employee’s spouse/partner, child or parent. This form allows you to request a letter of condolence from the president to offer support and express our sympathies during this difficult time.

The electronic form (Form 105-241) can be found in the College Forms Library. It is designed to streamline the process and ensure timely acknowledgment. We encourage you to use this form if you become aware of a bereavement within the NOVA community.
