If you need a Common Application / College Report completed, please fill out an Enrollment Verification Request Form (https://www.nvcc.edu/forms/pdf/125-094.pdf) and indicate in Step 4 “College transfer”. The completed Enrollment Verification Request form should include your name, empl ID, semester of verification, signature, and method of delivery.
Please complete “To The Applicant” portion of the College Report Form and attach both the College Report Form and Enrollment Verification Form and submit to Enrollment Services at any campus OR via email, enrollmentservices@nvcc.edu. If third party forms need to be completed, be sure to attach. It may take up to ten business days for your request to be processed.
